Proactive Steps to Protect Your Vision

Preventing vision loss in TED relies on early detection, careful monitoring, and timely intervention.

  1. Early and Accurate Diagnosis:

    • Recognize Early Symptoms: Be aware of the subtle initial signs of TED, such as persistent irritation, redness, pain behind the eyes, or mild changes in eye appearance (see Article 2).

    • Consult a Specialist: If you have these symptoms, particularly with a history of Graves' disease, seek out an ophthalmologist specializing in oculoplastics or orbital disease. These specialists have the expertise to accurately diagnose and manage TED.

  2. Regular Monitoring by an Ophthalmologist:

    • Visual Acuity: Checking your sharpness of vision.

    • Color Vision: Testing your ability to distinguish colors.

    • Visual Fields: Assessing your peripheral vision for any blind spots.

    • Eye Pressure: Measuring pressure inside the eye, as increased orbital pressure can sometimes affect the optic nerve.

    • Eye Movement: Assessing for limitations or double vision.

    • Proptosis Measurement: Tracking any changes in eye protrusion.

    • Teprotumumab (Tepezza): This targeted biologic therapy has shown significant efficacy in reducing inflammation and proptosis, thereby alleviating pressure on the optic nerve. It's a critical tool for preserving vision in active, moderate-to-severe TED.

  4. Surgical Intervention When Necessary:

    • Orbital Decompression Surgery: If medical treatments aren't sufficient or if optic neuropathy is severe and vision is threatened, emergency orbital decompression surgery may be necessary. This procedure creates more space within the eye socket by removing bone or fat, relieving pressure on the optic nerve and allowing the eye to recede.

    • Timing is Key: While most orbital surgeries are performed in the inactive phase of TED, optic neuropathy is one of the rare instances where urgent surgery during the active phase is indicated to save vision.

  5. Optimal Thyroid Hormone Control:

    • Work closely with your endocrinologist to ensure your thyroid hormone levels (TSH, T3, T4) are consistently within the normal range. While controlling thyroid levels doesn't directly treat the eye disease, poorly controlled hyperthyroidism can sometimes exacerbate TED.

  6. Lifestyle Modifications:

    • Quit Smoking IMMEDIATELY: Smoking is the single most significant modifiable risk factor for TED and greatly increases the risk and severity of vision-threatening complications. Quitting is crucial.
